How to refer a child

     

We welcome referrals from professionals, family members, parents, and young people themselves. We accept all children/young people at Acorns who have a life limited condition and are unlikely to live past their 19th Birthday.

Following the receipt of the referral form, the referral is discussed at a weekly panel meeting, where a decision is made for acceptance.

If a child/young person does not meet our Acorns’ criteria, the staff at the panel meeting will make every effort to signpost alternative support services for the family.

Emergency situations can be accepted by telephone and in non-emergency situations; the application form needs to be signed by a medical practitioner, or accompanied with a letter confirming diagnosis from the child’s/young person’s consultant.
