Find out how to refer a child or young person to Acorns.
Acorns welcomes referrals from across the region. To meet our criteria, children and young people need to be diagnosed with a life limiting or life threatening condition and have not yet reached their 18th birthday.
Families can be referred to Acorns by Paediatricians, GPs, Social Workers, Community Nurses or by themselves.
If you would like to make a referral please download our referral form here and email or post to the hospice local to the family as listed below.
We are also able to email a pdf copy of our referral form. If your referral is for end of life care, please ring the relevant hospice directly. Please note that the email addresses listed below are not monitored 24hrs a day or at the weekends. You may wish to phone the hospice to confirm that your referral has been received. To request a form, please email your nearest Acorns hospice using the email address listed below.
