Find out how to refer a child or young person to Acorns.

Acorns welcomes referrals from across the region. To meet our criteria, children and young people need to be diagnosed with a life limiting or life threatening condition and have not yet reached their 18th birthday.

Families can be referred to Acorns by Paediatricians, GPs, Social Workers, Community Nurses or by themselves.

If you would like to make a referral please download our referral form here and email or post to the hospice local to the family as listed below.

We are also able to email a pdf copy of our referral form. To request a form, please email your nearest Acorns hospice using the email address listed below.
